Consent Form 
THE USE OF TECHNOLOGY BY FACULTY MEMBERS AT OHIO UNIVERSITY 
You are invited to participate in a study investigating the use of technology at Ohio University. 
The researcher is pursuing a Ph.D. in Instructional Technology at Ohio University in Athens. To 
fully satisfy the requirements for this degree the researcher needs to complete a dissertation.  
INFORMATION
This study will require full time faculty members at Ohio University to respond to questions on 
academic and demographic information, teaching and learning strategies, and experience with 
technology.  Participants may spend about thirty minutes to answer these questions. The study will 
also involve interviewing faculty members on their changing roles and technology use. Each 
interview will take approximately twenty minutes and will be tape-recorded. These tapes will be 
transcribed verbatim. The data collection for this study will take place over a period of three 
months. There are no foreseeable risks for the participants involved with this study. You must be 
18 years of age or older to participate in this study. 
ANTICIPATED BENEFITS
The study will provide information that will help training and faculty development personnel, 
course designers, technical support services, and instructors at Ohio University to: 
Identify how technology is being used at the various colleges. 
The roles of faculty members toward the use of technology. 
Plan professional development to meet the needs of faculty in the colleges. 
Contribute to the body of knowledge concerning the use of technology in higher education. 
Provide a base line of information for future reference and further research studies in this 
area. 
CONFIDENTIALITY
This research will strive to protect every participant’s confidentiality. No results will be reported 
using real names or pseudonyms to protect the participants’ identities. All tape recordings and 
subsequent transcripts will be kept locked in a secure location and will be destroyed once the 
analysis is complete.
