STREET HAIL LIVERY PERMIT APPLICATION

Street Hail Livery Permit Application Form: must be completed and signed.
Social Security No. / Proof of EIN: If a corporation or partnership, you must submit an IRS issued CP-575 Notice or a 145-C
letter (provided as a replacement or verification letter to business). If a sole proprietor, you must submit proof of social security
number.

Zero outstanding judgments: to the TLC, NYS DMV Traffic Violations Bureau (TVB), Department of Finance (DOF) Parking
Violations and DOF Red Light Bureau (i.e., unpaid tickets) and Commercial Motor Vehicle Tax (CMVT).
***FEES can be paid by Money Orders, Certified Checks, Master Card, Visa, AMEX or Discover. Money Orders and Certified Checks
are payable to: NYC Taxi & Limousine Commission. A Street Hail Livery Permit must be placed into service with a licensed TLC vehicle
within (90) days of issuance. Exception: A Street Hail Livery Permit that is restricted to use with a Wheelchair accessible vehicle must be
placed into service with a For-Hire Vehicle or a Paratransit Vehicle within (180) days of issuance. Note: A maximum of two (2) credit cards
will be accepted to process a payment.
New Application Fee: Payable to the NYC Taxi and Limousine Commission (See method of payment below)
The fee to purchase a Street Hail Livery Permit for year two (2) is $3,000.
Inspection Fee: Five Hundred and Forty Dollars ($540.00): This fee covers the six (6) DMV\TLC inspections required during
the three (3) year term of the license. Inspection fees must be prepaid with each new and renewal application. Note: Paratransit
Vehicles licensed as Accessible Street Hail Liveries will be subject only to a visual inspection to assure that they are accessible
and that the accessibility features are operating in addition to a meter test.
PURPOSE OF THIS FORM:
This checklist is to be used in conjunction with the “Street Hail Livery Permit Application Form” to
submit an application to the NYC Taxi and Limousine Commission to obtain a Street Hail Livery Permit.
Current and Valid FHV Operator or Vehicle license: To purchase an SHL Permit you must have a current and valid FHV
Operator’s or Vehicle license for at least one
(1) year in good standing prior to the start of the second permit issuance period.
*In order to attach a vehicle to this permit, once granted, you must have your SHL base schedule an appointment for you. If
applicable, you may use your existing vehicle. On the day of your appointment,
you’ll need to bring your appointment form

SHL Service Requirement
–
A Street Hail Livery License must be placed into service with a For-Hire Vehicle within:
· 90 days of issuance if the SHL is a Convertible Non-Wheelchair Accessible Permit
· 180 days of issuance if the SHL is a Wheelchair Accessible Permit
A permit holder can apply for one extension provided that they submit an application for consideration PRIOR to their in service
requirement date (90 or 180 days from permit issuance). There will only be one (1) extension granted per permit to place a
vehicle in service. If the extension is granted, based upon the application and documents submitted, it will be for a MAXIMUM of
thirty (30) days only. No additional extensions will be granted.
